Job Description:

Ascension is seeking a dedicated and compassionate RN-Patient Navigator Oncology to join our faith-based healthcare team. This role is pivotal in guiding oncology patients through the complexity of their care paths, ensuring seamless coordination, and enhancing their experience within our network of healthcare services. The ideal candidate will embody our mission to innovate healthcare and provide personalized, compassionate care. Develop and implement patient care plans in collaboration with oncology patients, their families, and healthcare providers. Act as a primary point of contact for patients, providing education and support throughout their treatment journey. Coordinate multidisciplinary care meetings to discuss and evaluate patient progress and treatment plans. Monitor patient health status and outcomes, adjusting care plans as necessary. Facilitate access to healthcare services including scheduling appointments, arranging transportation, and navigating insurance issues. Advocate for patients' needs and preferences with healthcare teams to ensure comprehensive care. Maintain confidential patient records and document patient interactions and interventions appropriately. Strong communication and interpersonal skills Proficiency in electronic health record (EHR) systems In-depth knowledge of oncology care processes and best practices Ability to work effectively in a multidisciplinary team Strong organizational and case management skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license Oncology Certified Nurse (OCN) preferred
