Description:Work Schedule :Join the HBHC team beginning April or
June 2025. 60% FTE, day/evening shift. On-call responsibilities.
Weekend and holiday rotation. Hours are based off the operational
needs of the department. You will travel and work primarily in
patient's homes, your assigned work location will be 301 S.

hands-on acute care in the home setting for the Home-Based Hospital
Care patient population requiring the use of advanced assessment,
technical, teaching, and decision-making skills.We are seeking a
Registered Nurse (RN) :Work directly with patients and families in
the comfort of their home utilizing care plans, interventions and
equipment that are modeled after an acute care inpatient unit
setting.Perform comprehensive assessments of physical, functional,
psychosocial, and cognitive status of the Home-Based Hospital Care
patient utilizing interview observations and physical exam
techniques.Assess post-acute needs and ensure that appropriate
referrals to other services are made. Interdisciplinary
conferencing is a key component of this role's responsibility, and
thorough and timely documentation in EPIC is required.At UW Health,

Science in Nursing (BSN) PreferredWork ExperienceOne year of
relevant acute care or home health nursing experience
RequiredExperience in teaching self-care skills to patients and
families PreferredRecent, relevant home health experience
PreferredExperience using EPIC and virtual health platforms
PreferredCurrent IV therapy skills with knowledge of venous access
devices and ambulatory pumps PreferredLicenses &
CertificationsLicensed as a Registered Nurse (RN) in the State of
WI or holds a license issued by a jurisdiction that has adopted the
nurse licensure compact RequiredCurrent CPR/BLS Certification
RequiredCurrent motor vehicle liability insurance in accordance
with the UW Health "Motor Vehicle Policy" (Policy 1.17) and (1) A
valid Wisconsin driver's license and (2) Successfully passing a
driver's background check and (3) Two years of driving experience
