• Patient exam/evaluation (medical history, physical exam, review pertinent medical records/imaging, neurological Chiropractic exam and assessment)
• Diagnostic testing including X-ray imaging
• Diagnosis and Treatment Plan
• Report of findings
• Manual Adjustments
• Patient education re: treatment and ongoing care
• Documentation Qualifications:
  • Graduation from an Accredited DC program
  • Wisconsin License to practice Benefits:
