Job Description*Position: Resident Assistant / Medication Aide**Reports to: Administrator or Designee / RN / Shift Lead**Position Summary: Responsible for assisting the resident in all areas of care as assigned by RN Case Manager, Administrator or Assistant Administrator including, but not limited to: maintaining a safe and secure delivery of medications and health related items to Residents within the facility; assisting in developing, coordinating and performing tasks on the Care Plan in accordance with Resident preference and ability**Position Responsibility:

  • Adheres to and conveys community philosophy of maintaining elements of self-care and encouraging independence. Has awareness and practices in accordance with the regulations of the State of Wisconsin.
    • Receives report from previous shift and Medication Aide.
      • Reads the shift-to-shift communication log and shower schedule for information and changes before coming on shift. Documents in shift-to-shift report and personal care log (as appropriate) before going off shift.
        • Assists Residents with activities of daily living, environmental orientation, vital signs, first-aid wound care and communicates with outside health care professionals.
          • Prepares meals and assists residents to and from dining room while encouraging residents to find their own way.
            • Assists with serving meals and dining room duties as assigned.
              • Provides laundry and housekeeping services to residents as scheduled.
                • Demonstrates knowledge of infection control and universal precautions.
                  • Documents care given, adhering to facility policy and procedures.
                    • Maintains clean, neat, comfortable safe environment for Residents, staff and visitors, including housekeeping duties as assigned. Assures exits are alarmed at all times if needed.
                      • Reports deficiencies observed in Resident suites, clothing/linen and facility to the administrator. Completes Incident Report Form as needed for any unusual occurrence.
                        • Observes and reports any changes in resident physical or mental status and communicates to administrator and RN case manager.
                          • Is thoroughly familiar with fire, missing persons, and other emergency procedures. Follows facility policy and procedures detailed in the fire and disaster plans.
                            • Avoids loss, breakage and waste of supplies and equipment. Maintains a neat and orderly Medication Room.
                              • Wears name tag at all times.
                                • Assists Residents moving in or out as directed.
                                  • Performs resident activity programs as assigned.
                                    • Provides phone coverage as well as clerical and secretarial support as requested.
                                      • Maintains Care Plan and ensures implementation. May also participate in Care Plan reviews.
                                        • Orders, stores and assists with medications according to facility policy and procedure and current physician's orders.
                                          • Maintains current resident chart for each resident including, but not limited to, obtaining current physician orders, clarifying orders and month-end charting.**Employment Requirements:*
                                            • * Performs resident care with an awareness of dignity and individuality, with empathy, understanding and sincere concern.
                                              • Maintains acceptable attendance record; notifies administrator/assistant administrator if late or unable to work in accordance with personnel policies; follows assigned work schedule.
                                                • Requires minimal supervision. Is a self-starter.
                                                  • Organizes and utilizes time appropriately; sets priorities and accomplishes assigned tasks.
                                                    • Displays mature behavior and attitude in speech and action; demonstrates consistently pleasant demeanor, tone of voice, supports the philosophy and approach to care used by the facility.
                                                      • Keeps resident information confidential; respects resident's rights; respects privacy and right to self-determination of residents.
                                                        • Performs unpleasant tasks with grace; works cooperatively with supervisors and displays willingness to assist co-workers.
                                                          • Is able to communicate effectively verbally and in writing.
                                                            • Keeps work area neat and clean, helps to maintain appearance of the community.
                                                              • Abides by established policies and procedures of the facility. Looks for ways to improve facility functioning.
                                                                • Has ability to spend long periods on feet and to ambulate quickly.
                                                                  • Is free of non-treated communicable disease(s).
                                                                    • Is accurate, dependable, neat and thorough in assigned tasks.
                                                                      • Ability and willingness to learn quickly.
                                                                        • Participates in regularly scheduled in-service training as required by facility and state.**Training and Experience:Has familiarity with and willingness to work with elderly/disabled population. Nursing Assistant Certification, State of WI required certificates or the ability to complete the videos and pass the tests required to obtain the state required certificates; CBRF Medication Administration Certification required to pass medications.**Universal Precautions:Exposure to blood/bodily fluids is likely.**Physical Requirements:Has ability to do heavy lifting of residents (up to 80 lbs).*Job Type: Full-timePay: $16.00 - $18.00 per hourEducation:
