How you’ll make an impact in this role Improve and coordinate patient care. - Lead the comprehensive management of complex patient care journeys, leveraging evidence-based practices and creative solutions to drive optimal patient outcomes. - Drive continuous improvement in patient safety and clinical quality, actively minimizing hospital-acquired conditions and elevating patient satisfaction scores. - Coordinate seamless, cost-effective care plans for a diverse patient population, ensuring appropriate length of stay and the delivery of the most effective level of care. - Empower patients and families through tailored education and the implementation of health promotion strategies, enabling them to confidently manage acute and chronic illnesses. What minimum requirements you’ll need Licensure/Certification/Registration: - BLS Provider obtained prior to hire date or job transfer date required. - Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State. - Clinical Nurse Leader certification issued by the Commission of Nurse Certification (CNC) preferred. Education: - Diploma from an accredited school/college of nursing required and Master's degree required.
