Nurse Job Details - Employment Type: Travel RN - Healthcare
Setting: Home Health / Case Management - Assignment Length: 13
weeks (10/6/2025 – 1/3/2026) - Guaranteed Hours: 38-40 hours per
week - Shift: 8:00 AM – 4:30 PM (8-hour variable days; flexibility
based on census) - Weekend/Holiday: Weekend rotation every 3rd-4th
weekend; holiday rotation required - On-Call: 1-2 times during the
week and every 3rd or 4th weekend (at least once per month) -
Travel: Average 4.5 patient visits per day, up to 60 miles/day
depending on territory - Estimated Weekly Salary Range: $2100-$2250
Registered Nurse Qualifications - Associate Degree or Certificate
in Nursing required - Active Wisconsin RN license or Compact
Registered Nurse license - American Heart Association BLS
certification - Valid driver's license required - Minimum 2 years
of home health RN experience - Previous travel nursing experience
required - OASIS charting experience required; Epic EMR experience
preferred - OASIS certification is a plus - Strong time management,
adaptability, and communication skills Homecare RN Responsibilities
- Deliver skilled nursing care and case management to home health
patients in the Portage, Wisconsin area - Complete OASIS
admissions, episode management, and all required documentation in
EMR systems - Coordinate patient care with physicians and
interdisciplinary healthcare teams - Perform clinical skills
including wound care, IV therapy, port management, lab draws, and
chronic disease management - Participate in weekend and holiday
rotations, as well as on-call coverage for triage and urgent
patient needs - Float to other territories as needed based on
census and staffing requirements - Demonstrate compassion,
dependability, and a positive attitude in all patient interactions
