Job Description Job Description Care Coordinator Opportunity:
The Care Coordinator is an integral member of the interdisciplinary
treatment team serving youth and adults receiving services within
Acute Care Services, including Crisis Stabilization programs and
Inpatient Behavioral Health Hospital settings. This position is
primarily responsible for psychosocial assessment, safety and
crisis planning, care coordination, and comprehensive discharge
planning to ensure safe transitions to community-based services and
supports. The Care Coordinator collaborates closely with
psychiatrists, nursing staff, therapists, AODA providers, families,
and community partners to promote patient stability, recovery, and
continuity of care. What You’ll Get: The starting pay range for
this role is $26.92 - $29.31, based on education and experience.
This job is a full-time, in person position, working in the youth
and adult behavioral health hospitals. PTO, paid holidays, health

What You Bring: Required: Bachelor’s degree in Social Work or a
closely related field. More than 1 year of experience in behavioral
health, crisis, discharge planning or social work. Any experience
working with individuals with mental health or substance use
diagnoses. Valid Wisconsin driver’s license and use of a personal
vehicle. Preferred: Social work experience in a crisis or an
inpatient setting. Certified Social Worker in the state of
Wisconsin Substance Abuse Counselor or SAC-IT certification In
evaluating candidates for this position, North Central Health Care
may consider a combination of education, training, and experience
which provides the necessary knowledge, skills, and abilities to
perform the duties of this position. What a Care Coordinator does
at NCHC: Complete timely psychosocial assessments, safety plans,
crisis plans, and discharge summaries. Screen for danger to self or
others and assess for co-occurring mental health and substance use
disorders. Develop measurable treatment goals in collaboration with
patients and the interdisciplinary team. Provide individual,
family, and group social counseling services. Assist with safety
planning and crisis stabilization interventions. Monitor patient
progress and communicate updates to the interdisciplinary team.
Participate in daily rounds and interdisciplinary treatment
meetings to coordinate care. Obtain and integrate collateral
information from families, community providers, outpatient teams,
and other involved supports. Develop and implement individualized
discharge plans based on patient needs. Make appropriate and timely
referrals to community agencies, outpatient providers, and support
services. Facilitate communication and coordination with families,
guardians, and community providers to support continuity of care.
Provide clinical review and discharge documentation to insurance
providers as required. Serve as patient advocate to support safe,
ethical, and person-centered care decisions. Collaborate with
psychiatry, nursing, behavioral health professionals, case
management, CPS/APS, court liaison, and community agencies. Follow
mandated reporting requirements. Accurately document all
assessments, clinical contacts, treatment updates, referrals,
discharge planning, and care coordination activities in the
electronic medical record. Adhere to DHS, Joint Commission, HIPAA,
and organizational standards. Ensure services are delivered in a
respectful manner that upholds patient dignity, rights, and
person-centered care principles. Support a collaborative,
team-based approach to care. Perform other related duties as
assigned. Working Environment and Physical Requirements Frequent

hearing capabilities sufficient to effectively assess patient
presentation, respond to alarms or verbal cues, and maintain safety
within a behavioral health environment Exposure to individuals who
may be agitated, hostile, or behaviorally disruptive Community
travel as needed for continuity of care. Possible exposure to
communicable diseases and bodily fluids in an in-patient setting.
