Job Description:

Encompass Health is seeking a qualified and dedicated Pharmacist to join our team, delivering exceptional healthcare services, centered around rehabilitation and home health/hospice care. Our ideal candidate is committed to helping patients regain their independence by ensuring the safe and accurate distribution of medications. This role requires meticulous attention to detail, in-depth knowledge of pharmacotherapy, and the ability to provide compassionate care to our patients undergoing recovery. Review and validate prescriptions, considering therapeutic compatibility and patient safety. Consult with healthcare providers on medication therapy for patients. Educate patients and caregivers on medication regimens, potential side effects, and interactions. Manage the acquisition, storage, and dispersal of medications and medical supplies. Maintain accurate records of all controlled substances and pharmaceutical inventory. Adhere to all regulatory and safety standards in the preparation and dispensing of medications. In-depth knowledge of pharmacotherapy Exceptional organizational and detailed skills Strong verbal and written communication Ability to work collaboratively in a multidisciplinary healthcare team Proficiency in using pharmaceutical software and patient management systems Medical Insurance Dental Insurance Vision Insurance 401k Current and unrestricted Pharmacist license Board Certification in Pharmacotherapy, Geriatric, or similar fields preferred
