seeking a Registered Nurse (RN) Navigator to: - Work in
collaboration with numerous multidisciplinary teams to help
navigate patients and families through Pulmonary care. - Be a
conduit of information between inpatient and outpatient services
and pre- and post-surgical care for patients and families. -
Participate in continuous quality improvement initiatives. - Work
closely with Pulmonary providers, carry out coordinated plan of
care for patients, and be highly organized and great at

advancement. Qualifications - Bachelor's Degree in Nursing (BSN)
Required - Master's Degree in Nursing, Social Work, Public Health,
Healthcare Administration, or related field Preferred Work
Experience - 3 years of relevant clinical nursing experience
Required - 1 year of relevant experience in the specific patient
population being supported Preferred Licenses & Certifications -
Registered Nurse license in the state of Wisconsin Upon Hire
Required - Basic Life Support/CPR Upon Hire Required - Applicable
Oncology certification based on patient population Preferred Our
