Job Description:

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a dedicated Case Coordinator to join our vibrant team. This position involves managing multiple cases with the objective of fostering health, healing, and hope within the community. The ideal candidate will coordinate client care and connections to LSS services, including child and family services, addiction support, and community living assistance. This role is crucial in facilitating effective service delivery through detailed assessment, plan development, and regular follow-ups, ensuring alignment with our mission to support and enhance the well-being of our community. Conduct comprehensive assessments to determine client needs and service requirements. Develop, implement, and review service plans in collaboration with clients and other professionals. Coordinate and monitor multiple client cases simultaneously. Facilitate referrals to appropriate internal and external services. Maintain detailed records of client interactions, progress, and services provided. Ensure compliance with all federal, state, and agency regulations. Participate in team meetings and contribute to organizational goals. Strong organizational and multitasking skills Excellent communication and interpersonal skills Proven ability to manage cases independently Knowledge of social services and regulatory standards Proficiency in case management software and record keeping Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred Valid driver’s license and reliable transportation
