Job Description:
Seeking a dedicated MDS Coordinator (LPN/RN) for a skilled
nursing facility in Carlisle, PA.
Ensure accurate and timely resident assessments, maintaining
compliance with regulations and facility policies.
Monitor residents regularly for condition changes and coordinate
assessments such as MDS, quarterlies, CAAs, and care plans.
Collaborate with interdisciplinary teams including Social Services,
Activities, and Dietary to develop and update care plans.
Maintain the MDS schedule, submit data to the state weekly, and
keep detailed records.
Qualifications include active PA RN or LPN license and at least one
year of MDS experience in long-term care.
