  • Job Details
  • Evaluates and treats all referred Physical Therapy patients according to physician orders.
  • Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients.
  • Performs initial patient evaluations including administering appropriate tests/measures, identifies impairments and functional limitations.
  • Establishes an initial plan of treatment in conjunction with the patient/caregiver.
  • Selects and administers treatment strategies based on current physician orders and established treatment plan(s).
  • Delegates appropriate tasks and supervises the PTA or HHA.
  • Identifies functional improvement and goal attainment on a regular basis, supported by documented subjective and objective changes.
  • Updates treatment plans and goals per department standards.
  • Completes documentation following department standards and timeframes, reflecting skilled intervention and supporting therapeutic interventions as reasonable and necessary.
  • Achieves productivity per department standards and completes job duties efficiently and effectively.
  • Complies with applicable rules and regulations in Home Health Care.
  • Demonstrates accuracy with OASIS and adheres to the National Patient Safety Standards.
  • Demonstrates responsible utilization of resources for each patient and actively participates in department PI activities.
