Job Description:

Seeking a Social Worker to support patient discharge and identify barriers to care in a hospital setting.

The Social Worker will work with patients, families, and care teams to ensure safe discharge and address social determinants of health.



Job Responsibilities:

• 
  
• Interview patients, assess priorities, and document case activity.
  
• Consult with care teams and community resources to support patient treatment.
  
• Assist patients/families with obtaining community assistance by referral to proper resources.
  
• Develop and maintain working relationships with community health, welfare, and social agencies.
  
• Participate in team discussions regarding opportunities for improvement in standard work.
  
  
  
  
  Requirements:
  
  • 
    
  • Master's degree in Social Work.
    
  • No experience required.
    
    
    
    
    Physical Requirements:
    
    • 
      
    • Frequent reaching, sitting, standing, and walking.
      
    • Frequent use of vision and depth perception for distances near and far.
      
    • Frequent use of hearing and speech to share information through oral communication.
      
    • Occasional bending, stooping, climbing, twisting, and gripping.
      
    • Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
