Job SummarySSM Health is seeking a Registered Nurse - Case Manager to join our team. As a key member of our care coordination team, you will play a vital role in ensuring patients receive the best possible care and support throughout their hospital stay.Responsibilities

• Assess patients' discharge planning needs and develop individualized plans to ensure a smooth transition to the next level of care.
• Collaborate with interdisciplinary teams to identify and address barriers to discharge, including social determinants of health and access to community resources.
• Utilize evidence-based practice and clinical expertise to make informed decisions about patient care and discharge planning.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
• Participate in quality improvement initiatives to enhance patient outcomes and reduce readmissions.Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of employment.
  • At least 3 years of acute care experience, preferably in a case management or care coordination role.
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
