Work Schedule:
100% FTE, days. You may be eligible for up to a $3,000 sign-on bonus. This is a hybrid position. When in person, you will work at 2402 Winnebago Street in Madison, WI.

Working as an independent health care provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to adult patients while working collaboratively within a multidisciplinary health team.
We are seeking a Behavioral Health Clinician to:

• Collaborate with other medical professionals in a fast-paced environment.
  
• Provide brief evidence-based interventions.
  
• Provide short, but intense follow-up.
  
• Connect patients to resources.
  
• Use outcome-based models to track patient improvement.

    Education:
    Minimum - Master's degree in social work, psychology, or counseling
    Work Experience:
    Minimum - Two (2) years of experience in a health care environment in a patient/client contact role
    Licenses and Certifications:
    Minimum - Relevant license and/or certification or license/certification eligible as a Mental Health provider as required in the State of Wisconsin (i.e., Psychologist, Licensed Professional Counselor-LPC, Licensed Clinical Social Worker-LCSW, Licensed Marriage and Family Therapist-LMFT)
