Our goal is to see patients with realistic time, to provide the right level of care to make the clinical impacts outlined below:The Physician Team Leader (Pod Leader) acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team. Success in Value Based Care changes the demands placed on primary care.
