Job Description:

for providing medication therapy management services in
collaboration with other providers to assure that drug therapy is
safe, effective, efficient and cost-effective. The Clinical and
Sterile Products Pharmacist shall be responsible for Sterile
Products (USP 795/797/800) oversight Completion of the ASHP sterile
products certificate Oversee MFT/GFT testing schedules every six
months and required sterile products competencies for staff
Pharmacy and Therapeutics Committee clinical reviews and
presentations to the committee Active involvement in the pharmacy
residency program Drug shortage management, along with
recommendations and communication to the healthcare team Clinical
education and training oversight for the pharmacy department. This
shall include: Policy and procedure development and updates
Oversee, provide and coordinate education to other healthcare
providers (presentations at competency days, etc) Oversee and
coordinate training schedules for new employees Coordinate, oversee
and/or provide education on any new clinical initiatives Other
pharmacy services provided include but are not limited to
interdisciplinary patient care rounds, admission and discharge
medication reconciliation, medication therapy monitoring,
medication orders management, patient education, drug information,
drug dosing, and various scholarly activities. The Pharmacist
performs both clinical and distributive tasks as necessary. The
Pharmacist will provide training and education to pharmacy resident
and students through practical and didactic experiences. Incumbent
acts independently within the scope of their license and areas of
expertise with the help of general policies, procedures and
practice guidelines with support from the Lead Pharmacist and
Pharmacy Director. The pharmacist ensures that patient care is
maintained through compliance with the department's policies, rules
and regulations promulgated by the Board of Pharmacy, and other
applicable regulatory bodies. We provide pharmacy services in the
Hospital, Specialty/General Clinics, Nursing Home and work
collaboratively with our Outpatient and Ambulatory Pharmacy teams.
The incumbent will be expected to handle hazardous drugs
(chemotherapy) with appropriate primary and secondary engineering
controls. Work Schedule: 40 hours per week. Shifts scheduled
between 0700-1900, Monday-Friday, Weekends 0800-1700.
Weekend/Holiday/On-Call rotation is required. Hours may vary based
on operational need of the department. RAMC offers a competitive

disability. Qualifications Education Minimum: PharmD degree or
equivalent Preferred: Completion of an ASHP Accredited Residency
training program. Work Experience Minimum: One year of clinical
patient care and order management experience. Preferred: Three
years of institutional or Community Hospital or completion of an
ASHP-accredited residency training program. Licenses &
Certifications Minimum: Licensed or eligible for licensure to
practice pharmacy in the State of Wisconsin. Preferred:
Certification by the Board of Pharmacy Required Skills, Knowledge,
