If so, iCare is looking for a Nurse Practitioner join our growing Dane County team in the Family Care Partnership (FCP) Program! The NP is responsible for providing advanced level case management and care coordination for Family Care Partnership members in collaboration with the Interdisciplinary Team (IDT), the IDT Supervisor. The NP also provides clinical and administrative support for initiatives to achieve member health outcomes related to program and company objectives.ESSENTIAL DUTIES and RESPONSIBILITIES:Member of the IDT

  • Understand the Family Care model and work within these parameters
  • Work collaboratively with the IDT Staff and Supervisor to provide clinical expertise
  • Provide assessment for new members as needed and members experiencing acute care needs and collaborate with the Supervisor and IDT on the findings and plan development
  • Discern member needs that require NP level intervention, work with Supervisor and IDT to create a plan to address them
  • Provide assessment and intervention for identified members from the Supervisor and IDT, collaborate and communicate on findings and plan development
  • Ensure appropriate care management for members identified as needing NP involvement.
  • Provide clinical expertise as needed in routine team meetings and case staffingProgram Support Roles
    • Accept assignment to special areas of focus for program and companywide initiatives
    • Provide clinical expertise to leadership in the development of strategies to achieve member health and company objectives

        • Master's Degree in Nursing.
        • Possession of a licensure or eligibility for licensure in the State of Wisconsin as a Registered Nurse, Advance Practice Nurse Prescriber (APNP) licensure, and national NP certification in geriatrics, adult or family practice.
        • Strong assessment and care planning skills required along with the ability to provide complex care coordination.
        • Ability to effectively communicate thoughts, ideas, and information both orally and in writing.
        • Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects.
        • Strong interpersonal skills and ability to effectively interact with providers, members and co-workers from a variety of different backgrounds and experiences in a professional and courteous manner.
        • Ability to work effectively as a team member and cooperate in achieving company goals.
        • Ability to think creatively and solve complex problems using best practice standards within the context of a managed care environment.
        • Must be able to travel to any location in Dane and Sauk counties with some travel possible in other counties within Southeast WI.
        • A personal vehicle, valid State of Wisconsin motor vehicle operator's license and conformity with insurance coverage limits are required.PREFERRED QUALIFICATIONS:
          • Previous home health, managed care or case management experience.
          • Previous experience working with persons in any of all of the target member populations: (frail elderly, physically disabled, developmentally disabled) in a health care setting.
