• Function autonomously to care for patients with complex, acute, critical and chronic health conditions.
• Diagnose and treat acute infections, injuries and illnesses
• Wound care management based on current standard
• Provide occasional in-servicing on wound care topics for facility staff
• Prescribe medications including the frequency and dosage
• Work individually and in interdisciplinary teams of resources and consultants
• Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
• Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
• Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care Skills
• Demonstrated problem-solving and workflow management
• High level of clinical proficiency and excellent decision-making skills.
• Ability to work independently and as an effective member of a healthcare team.
• Excellent oral and written communication skills.
• Ability to effectively collaborate and build relationships with patients, families and the all care providers
• Proven organizational skills and ability to prioritize effectively.
• Strong computer skills with knowledge and experience with Electronic Medical RecordsQualifications
  • Board Certified as a FNP
  • DEA Substance License required
  • BCLS/ACLS certification required
  • Prior APN experience preferred
  • CWCA preferred
