Job Description:
The clinical pharmacist at UW Health provides pharmacy patient
care services as part of an integrated pharmacy practice model and
a member of interdisciplinary care teams. Responsibilities include:
interdisciplinary patient care rounds; pharmacist medication
histories for all admissions; medication reconciliation; pharmacist
discharge teaching and counseling for all discharges; member of the
resuscitation team; prospective order review; clinical monitoring;
precepting students, interns and residents; lead/participate in
quality improvement projects; scholarly activity; oversight of
medication use systems; and drug policy development. Pharmacist
practice is supported with pharmacy automation, pharmacy
technicians, a fully integrated electronic health record,
professional development programs and a pharmacist practice
advancement and recognition program (clinical career ladder).
