The Opportunity
Dean Health Plan is searching for a progressive thought leader
to join our organization as the Director
of Care Management. This role is responsible for providing strategic oversight of our care and disease management teams
in both our Wisconsin and Missouri markets.
The right individual is capable of supporting system growth
initiatives in multiple markets and excels in evaluating program effectiveness of common chronic conditions and complex
case management. The incumbent will collaborate with leadership throughout the integrated delivery network to motivate
and align providers on pay-for-performance arrangements.

offers the right person the chance to make

Requirements for the position
degree in healthcare.
Director level over CM.
