Job SummaryAs a RN Case Manager Weekend Option, you will play a vital role in ensuring patients receive seamless care and support throughout their hospital stay. This position requires a skilled and compassionate nurse who can assess, develop, and implement discharge plans that meet the unique needs of each patient.Key Responsibilities

• Assess patients' discharge planning needs and develop individualized plans to ensure a smooth transition to the next level of care.
• Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact patient outcomes.
• Participate in the utilization management process to ensure patients receive the most appropriate level of care.
• Act as a liaison between the hospital and post-acute providers to ensure seamless transitions of care.Requirements
  • Graduate of an accredited school of nursing.
  • Three years' acute care experience.
