Job Description:

ESSENTIAL RESPONSIBILITIES1. Coordinates clinical functions and personnel; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations (Conditions of Participation)2. Understand current Hospice Conditions of Participation and develop and implement clinical changes on agency delivery of care model3. Able to perform/coordinate initial certifications and ongoing certifications as needed4. Provides direction to subordinates, in an effort to ensure quality, compliance with the Hospice Plan of Care and assessment and reassessment of patients' needs and continuity of services by appropriate health care personnel5. Direct and evaluate efficiency and professional practice of nursing, intake, and agency staff6. Monitors QAPI Program and assures appropriate corrective measures are performed7. Promotes compliance with all fiscal intermediary and/or other third-party payers, through educations, coaching, and other assistance as necessary8. Monitors systems and identifies problem areas to Administrator both verbally and through written reports9. Meets mandatory continuing education requirements of the agency/licensing board10. Promotes and educates regarding the concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease and ensure adequate orientation and staff development needs are met through collaboration with Oakwood clinical education programming.11. Work with and ensure support of the Sales and Marketing Staff to progress the agency in the community and across Oakwood campuses, build census and grow the agency in line with our performance goals.12. Review and analyze agency activities and provide data to aid in planning risk management and improve services13. Work with HR to recruit/hire and orientate qualified personnel and resolve employee issues14. Assume leadership of care management team15. Establish work schedules and assignments for staff, according to workload, space, and equipment availability. Supervise staff visits and staff care delivery as needed.16. Maintain awareness of advances in nursing, government regulations including the development, revision and implementation department policies and procedures as required17. Monitor the use of diagnostic services and other care services (e.g., pharmacy) to ensure effective use of resources and assess the need for additional staff, equipment, and services18. Coordinate with other department directors in developing, enhancing, and maintaining computerized record management systems to store/ process data such as personnel activities, information and produce reports as appropriate19. Implement agency objectives and evaluative or operational criteria20. Prepare staff activity reports to inform management of issuesESSENTIAL QUAIFICATIONS1. Clinical competency to assess and provide accurate care planning and review, of a diverse range of hospice and palliative care cases.2. Managerial competency to direct a caregiving staff of up to 30 individuals across a caseload averaging up to 80 patients.3. Organizational capacity to build internal relationships with key campus stakeholders (Social Services, Nursing, Administration, Health and Wellness, etc.) to facilitate admissions, discharges, and integrated services among campus residents, while on caseload.4. Strong survey/accreditation history of maximum, ongoing compliance. Experience with ACHC preferred.5. Flexibility to work remote, in-office and within patient locations.6. Proven history of ability to recruit clinical staff and to maintain staff - limited turnover with high productivity.7. Excellent communication skills, both oral and written.8.Excellent interpersonal skills, including the ability to deal with people from a wide range of backgrounds, ages and temperament.9. Ability to use discretion concerning sensitive information and adhere to HIPAA.10. Ability to work on own initiative within clearly defined parameters.11. Ability to carry out designated tasks including reporting outcome of actions taken.12. Customer service abilities including effective listening skills13. Good organizational and time-management skills.14. Comfortable presenting in front of professional groupsEDUCATION, EXPERIENCE AND/OR TRAINING1. Bachelor's Degree or Associate Degree in Nursing2. Wisconsin license, unrestricted and current, as a Registered Nurse3. Hospice and Palliative Care RN certification a definite plus4. Five years of progressive management/leadership experience5. Two plus years of experience in Hospice - accredited, Medicare certified agency level6. Knowledge and experience with relevant industry software
